As you can see, for some reason every line ends with an equals sign, including the line-broken link to fix the application. I tried to edit and fix the link, but without success. I can only guess what it is supposed to look like.Submitted application rejected

Try this while logged into the AppDB:

It works for me, but I'm an admin and it shows the full admin list for me, so I'm not sure what it will show for a non-admin account. If that works, what I did is remove all the end-of-line equal signs, and the "3D" and "amp;" strings from the URL.
